Health Office

The Health Office is located on the first floor of Boland Hall. It is staffed by a registered nurse and nurse's assistant between the hours of 7:30am—3:30pm Monday through Friday during the school year. The school nurse is available to dispense medications and evaluate illnesses and injuries.

Click on one of the links below to download the necessary forms. Completed and updated health forms are a requirement for all students every year. Forms must be returned by July 1, 2013. All medication must be registered with and dispensed by the school nurse.
