Please use the form below to submit a nomination for the Athletic Hall of Fame. You may nominate an individual or a team at anytime. In the event your nominee fails to earn election in a given year, there is no need to re-submit the nomination form the following year, as the Selection Committee retains nomination forms indefinitely.
